Any knowing falsification by an applicant or individual other than an authorized school official may result in denial of admission or dismissal. This form should not be used to upload medical documentation or health records.

For high school application materials: Documentation may only be submitted by a school counselor using this form.

For college transcripts: Official college transcripts may only be submitted using this form by an authorized sender of college transcripts at the college from which the transcript is produced. Typically, this is the office of the Registrar. College transcripts received from school counselors and academic advisors using this form are considered unofficial.
